Eversense
Eversense by Senseonics, Inc. produces the E3 Continuous Glucose Monitor (CGM) Monitor which continually measures glucose levels for up to 180 days in people with diabetes.

Data Collection
Eversense CGM collects:
Account information when you download and install a Senseonics Mobile App.
Transmitter serial number and Sensor serial numbers.
Synched devices about current and historical glucose values and any additional health-related personal data is transmitted from your Device to Senseonics servers.
Customer Care communications.
Information collected about health care professionals (HCPs) managing a patient who uses the Eversense system.
Information collected about Patients’ friends and/or family from the Eversense NOW Remote Monitoring App.
Data Sharing
Data may be shared with:
Affiliates - Senseonics’ corporate parent, subsidiaries, and affiliates.
Business partners and service providers - for example, customer care services, customer relation managmenet services, teleperformance, etc.
Government agencies, regulators, and professional advisors - in the case of legal obligations.
Organizations involved in business transfers - in the event of a merger, reorganization, dissolution or similar corporate event, or the sale of all or substantially all of our assets.
Third-party systems as directed by patients including your HCP through Eversense DMS Pro System, the Eversense NOW Remote Monitoring App, and other third parties such as Diasend, mySugr, Apple Health or Glooko.
Privacy Rights
You can review, correct, update, suppress, or delete personal data that you have provided by maintaining it your account on the Eversense app or contact the company at dataprivacy@senseonics.com. You may also deactivate your Senseonics account by contacting that address.
Data Protection
Senseonics and Eversense have implemented “prodcedures to deal with any suspected breach of personal data” and “appropriate information security policies, rules and technical measures to protect the personal data” that users provide.
